Developed for less invasive treatment and to expand the usage of trans-radial approach the Sheathless Eaucath PTCA Guiding Catheter is one of the latest innovations from Asahi Intecc. It is highly recommended for ad hoc PCI as it allows PCI to be performed with a smaller puncture site, thus realising minimal invasive treatment.

Features

Case Presentation By Dr. Douglas Fraser, Manchester Royal Infirmary, United Kingdom

A sixty-year-old man was admitted with an acute coronary syndrome with a raised troponin and anterolateral ECG changes. Coronary angiography via a 6F radial sheath and Judkins diagnostic catheters showed a culprit bifurcation lesion in the LAD. He had no known prior history of coronary artery disease. The bifurcation disease involved a long severe stenosis in the proximal into the mid LAD that involved a large diagonal branch that also had a long and severe segment of disease extending from its origin.

Case: The pattern of the disease mandated a 2 stent strategy. Crush stenting was selected as the only strategy that would ensure patency of both LAD and diagonal vessels. This required a 7F or larger guide catheter. Conventional 7F guide catheters require a sheath and such large sheaths may be poorly tolerated in the radial artery.
Intervention: We therefore selected a 7.5F PB Sheathless guiding catheter, and following wiring of both vessels, the LAD and diagonal were predilated with 2.5mm balloons. Distal to the bifurcation, a 2.5x28mm Promus stent was placed in the LAD and a 2.5x28mm Promus stent was placed in the diagonal. At the bifurcation a 2.5x15mm Promus stent was placed in the diagonal with a 3mm tail extending proximally into the LAD; this was overlapped by a 2.75x28mm Promus stent in the LAD that was placed across the bifurcation.

A further 2.75x18mm Promus stent was then placed in the proximal LAD. The diagonal was then rewired and a final kissing balloon dilation was performed.
